29 CFR 1926.20(b)(2): A program was not provided for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employer: a) Job site, 4401 Ohio River Boulevard, Bellevue, PA: On or about March 25, 2026 during roofing work, the employer did not ensure a system of hazard identification and correction was implemented at the worksite. Deficiencies noted included, but were not limited to, leaning too far beyond ladder side rails, not having 3 points of contact with the ladder, grinding/cutting without eye/face protection and failing to use fall protection when needed.
