29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): Ardagh Metal Packaging USA Corp. - Huron, Ohio: On or about March 20, 2026, employees were exposed to injury from rotating parts and pinch points of the Line 6 decorator/oven in that the machine was not locked out prior to replacing the machine's pin chain. The employer did not ensure that energy control application steps, to control the electrical energy source for the pin chain, were implemented prior to the servicing tasks - the employer did not: - Turn off or shut down the equipment. - Physically operate energy isolating devices to control energy sources and affix lockout device(s). - Ensure that all potentially hazardous stored or residual energy was relieved, disconnected, and rendered safe. - Verify that isolation and deenergization of the machine or equipment had been accomplished. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: Ardagh Metal Packaging USA Corp. - Huron, Ohio: On or about March 26, 2026, the employer's periodic inspection of energy control procedures for the Line 6 pin oven did not include a demonstration of the procedures while an authorized employee performed servicing and/or maintenance activities, to ensure that lockout procedure requirements of 29 CFR 1910.147 were being followed. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
