Location: Job site; On or about 03/27/2026, employees were permitted to use a job made wooden scaffolding system which was constructed of five (5) separate sections of 2"x12" wooden planks containing several knots and other imperfections. These scaffold planks were not graded by a recognized lumber grading association for this use and would not be capable of supporting 4 times the maximum intended load.
