1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s): Location: 127 Bay St, Hull, MA On March 20, 2026, a worker was exposed to fall hazards of up to 25 feet to the ground below while conducting roofing activities where fall protection was not used to protect workers on the roof and on a pump jack scaffold. The employer did not have a program to conduct and document frequent and regular inspections of their worksites to ensure that workers were protected from falls.

29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee shall be protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. Location- 127 Bay St, Hull, MA On March 20, 2026, workers were exposed to fall hazards of up to 20 feet to the ground below while using pump jack scaffolding to conduct siding activities where fall protection was not used.
