29 CFR 1926.404(b)(1)(iii)(C): Where an assured equipment grounding conductor program was being utilized, the employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage. (a) On or about December 9, 2025, employees working at a new residential construction site while utilizing a electrical cords that exhibited strain relief.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b): (a) On or about December 9, 2025, employees doing decking work on a single-family residential house and were exposed fall hazards to the lower level. The Viane Brito was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1703496, citation number 2, item number 1 and was affirmed as a final order on November 23, 2023, with respect to a workplace located at 2513 Buttonbush Lane, Garland, Texas 75042.
