29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: a. On or about April 1, 2026, an employee was working within an 8 foot deep excavation which was not adequately sloped or shored where water was accumulating without provisions to mitigate excavation wall collapse hazards. Additionally, the employee was not provided with a ladder or safe means of egress.
