29 CFR  1926.451(f)(6): Scaffolds were erected, used, dismantled, altered, or moved such that they, or any conductive material handled on them, could have come closer than the minimum distances established for specific voltages.  JOBSITE/290/292 SHERMAN AVENUE, NEW HAVEN, CT 06511: On/or about April 2, 2026, the employees were exposed to the primary power line while working on a metal frame scaffold system, where the scaffold was erected approximately five feet from the primary power line while employees were repairing a chimney.

JOBSITE/290/292 SHERMAN AVENUE, NEW HAVEN, CT 06511: On/or about April 2, 2026, employees were exposed to fall hazards from heights up to 24 feet to the ground while working on a metal frame scaffold system where the scaffold system was not equipped with guardrails or fall protection systems.
