29 CFR 1926.416(a)(1): The employer permitted employees to work in close proximity to energized, unguarded, and ungrounded electric power circuits that the employee could contact in the course of their work:    (a) Residential structure, north side; On or about March 31, 2026, workers performed roofing activities on the upper and lower roof surfaces in close proximity to the home's energized and exposed electrical service mast and service?drop conductors supplying 120/240?volt power to the residence, creating the potential for inadvertent contact with energized parts.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  (a) Residential structure rooftop, north side; On or about March 31, 2026, workers were not adequately protected from falls while engaged in roofing replacement activities from the rooftop of a two-story single-family home at approximate heights ranging between 10-feet, 2-inches to 24-feet, 10-inches above ground-level.
