29 CFR 1904.10(b)(4): Incidents of recordable Standard Threshold Shifts were not recorded on the OSHA 300 Log of Injury and Illness when the affected employee(s) were not retested within 30 days of the first test:  a.  Employees were exposed to hazardous noise and suffered recordable standard threshold shifts as documented on November  20, 2025.  The employees did not have follow-up testing and the incidents were not recorded on the 2025 OSHA 300 Log.

29 CFR  1904.41(a)(1)(i): The establishment had 20 or more employees but fewer than 250 employees at any time during the previous calendar year, and the establishment was classified in an industry listed in appendix A to subpart E of this part, and the employer did not electronically submit information from OSHA Form 300A Summary of Work-Related Injuries and Illnesses to OSHA or OSHA's designee by the specified date:  a. Tatum Brothers Lumber Company, Inc. located at 22796 CR 200A, Lawtey, Florida: On or about April  7, 2026, and times prior thereto, the employer failed during calendar year 2025, to electronically submit information from the OSHA Form 300A or equivalent by March 2, 2026. The establishment employed approximately 50 employees and was classified in the North American Industrial Classification System as 321113 during calendar year 2025.
