29 CFR 1926.304(d): Portable, power-driven circular saw(s) were not equipped with a guard above and below the base plate or shoe:  a) On or about April 7, 2026, employees were exposed to laceration and amputation hazards while operating portable circular saws. The employer allowed the use of circular saws with the lower blade guards shimmed back using pieces of wood, preventing the guards from functioning as intended during framing activities.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) On or about April 7, 2026, the employer exposed employees to fall hazards of approximately 11 feet to the lower level, in that the extension ladders provided did not have side rails which extended at least 3 feet to allow for safe access to each level.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:    a) Deck: On or about April 7, 2026, the employer exposed employees to fall hazards of approximately 12 feet to the lower ground level, in that a fall protection system was not provided to employees engaged in framing activities.  Luis Alberto Chilel was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1778985, citation number 1, item number 2, and was affirmed as a final order on 10/30/2024, with respect to a workplace located at 1881 2nd Ave, Lots 502, 503, 504 Vela Park Subdivision, Decatur, GA 30032.
