29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: a) Jobsite, 156 Industrial Boulevard, Everett, PA 15537: On or about March 31, 2026, Linebar Resaw components, including the Pop-Up Roll Case Assembly and the Inspection Deck Assembly, were not locked out each time employees step onto the equipment to straighten out cants. Employees were exposed to caught-by and crushed-by hazards created by the movement of hydraulically activated components.
