29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed Utulei, Route 001: On or about April 10, 2026, employees improperly exited the trench by leaning a closed A-frame ladder against the sidewall of an excavation.
