29 CFR 1910.147(c)(4)(i): Procedures were not documented or utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  a) On or about 04/09/2026, in the Forming Area, energy control procedures (Lockout/Tagout LOTO) were not documented or utilized when employees performed servicing and/or maintenance activities such as but not limited to setting up dies on the Green di-acro PB06 and PB05 and Niagara PB17 and PB18 press brakes. Employees were exposed to amputation hazards.  b) On or about 04/09/2026, in the Machining Area, energy control procedures (Lockout/Tagout LOTO) were not documented or utilized when employees performed servicing and/or maintenance activities such as, but not limited to changing the blade on the Cosen Saw. Employees were exposed to amputation hazards.   ABATEMENT CERTIFICATION REQUIRED

29 CFR 1910.212(a)(1): One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) On or about 04/09/2026, in the Forming Area, employees were exposed to amputation hazards when operating the Green di-acro PB06 and PB05, Niagara PB17 and PB18, and AMADA RG50 Press Brakes without point-of-operation guarding.   b) On or about 04/09/2026, in the Forming Area, employees were exposed to amputation hazards when operating the Green di-acro PB06 and PB05 without guarding the foot pedal.  c) On or about 04/09/2026, in the Machining Area, employees were exposed to rotating parts hazards when operating the Millport SJ-1540G Lathe without guarding the chuck.   d) On or about 04/09/2026, in the Machining Area, employees were exposed to nip point hazards when operating the belt sander without the side guard.
