29 CFR  1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:   (a) Throughout the excavation; On or about April 14, 2026, the employer did not ensure that employees working in an excavation up to 4 feet 8 inches in depth to construct foundation formwork were provided with a proper means of egress located within 25 feet of all work locations.  Pember Companies, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1675296, citation number 1, item number 1, group number a, and was affirmed as final order on August 29, 2023, with respect to a worksite located at 2405 N. Clairemont Ave, Eau Claire, WI 54701.
