29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: a.) 3500 Neville Road, Pittsburgh, PA 15225, facility : On or about March 26, 2026 at the fabrication area, employees operating a bradbury hydraulic 26 inch long folder were exposed to point of operation hazards. The bradubury hydraulic 26 inch long folder was not equipped with adequate guarding such as barriers and/or presence sensor devices to prevent employee hand placement from entering the pinch point during operation.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a.) 3500 Neville Road, Pittsburgh, PA 15225, facility : On or about March 26, 2026 at the fabrication area, employees operating a bradbury hydraulic 26 inch long folder were not provided machine specific training on guarding requirements. Employees were operating equipment without proper guard use or safe hand placement, exposing employees to crushing hazards.
