29 CFR 1910.178(o)(1): The employer did not ensure that only stable or safely arranged loads were handled: On or about 4/6/2026, at 1300 S. Bishop Ave in Rolla, MO, the Heart of the Ozarks Screen Printing, LLC did not ensure that only stable loads were handled. A large piece of equipment was being moved with a Hyster forklift. The load was unstable and fell off of the forklift's forks crushing a nearby employee.
