Location: 28 Billings St. Sharon MA. On or about 16 April, 2026, , multiple anchor points installed on the roof were observed with screws not fully driven or secured. These anchor points were not installed in accordance with manufacturer instructions and therefore could not be assured to support required loads.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: Location : 28 Billings St. Sharon MA. On 16 April, 2026, the subcontractor stated to the CSHO that employees had not received fall protection training. Employees conducted roofing operations at heights of up to 11 feet without knowledge of proper use of fall protection equipment, anchorage, or hazard recognition.
