29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s) Location 28 Billings St. Sharon, MA. On 16 April, 2026, the General Contractor (GC) supervised a subcontractor performing roofing work at a two?story residential structure. Multiple uncontrolled hazards were present, including: Workers working on the roof with no fall protection in use. A worker operating a pneumatic nail gun without required eye protection. Anchorage points that were installed incorrectly (screws not fully seated). Ladders used for access that failed to extend 3 ft above the landing surface. The GC's representative responded to the site only after OSHA arrived and was unable to provide any inspection records, checklists, or documentation demonstrating that a competent person had conducted frequent and regular inspections as required.
