(a) Entire Facility : On or about April 15th, 2026, the employer failed to provide an emergency eyewash station within the immediate vicinity of employees working with and around a Toyota Electric Forklift Model 7FBEU15 , Toyota Rider Pallet Jack Model 8HBE40, Toyota Electric Tow Truck 8TB50, Big Joe S22-62 Electric Straddle Stacker and Enersys NEXSYS Model NIP3-JN-4Y 480DC Forklift Battery Charger. (b) Expansion Building 163 Hazard Materials Room: On or about April 15th, 2026, the plumbed eye wash station did not provide adequate water pressure when activated.

If the load being carried obstructs forward view, the driver shall be required to travel with the load trailing.
