29 CFR  1910.151(c): Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body shall be provided within the work area for immediate emergency use.  (a) Entire Facility : On or about April 15th, 2026, the employer failed to provide an emergency eyewash station within the immediate vicinity of employees working with and around a Toyota Electric Forklift Model 7FBEU15 , Toyota Rider Pallet Jack Model 8HBE40, Toyota Electric Tow Truck 8TB50, Big Joe S22-62 Electric Straddle Stacker and Enersys NEXSYS Model NIP3-JN-4Y 480DC Forklift Battery Charger.  (b) Expansion Building 163 Hazard Materials Room: On or about April 15th, 2026, the plumbed eye wash station did not provide adequate water pressure when activated.

29 CFR  1910.178(n)(4): The industrial truck driver shall be required to slow down and sound the horn at cross aisles and other locations where vision is obstructed. If the load being carried obstructs forward view, the driver shall be required to travel with the load trailing.  Entire Facility: On or about April 15th, 2026, employees operating a Toyota Electric Forklift Model 7FBEU15 , Toyota Rider Pallet Jack Model 8HBE40 and Toyota Electric Tow Truck 8TB50 , while driving through intersections and aisleways, did not sound the horn or slow down to come to a complete stop.

Area BB10: On or about April 15th, 2026, the employer did not ensure removal of the Toyota Rider Pallet Jack Model 8HBE40, that was not in safe operating condition due to exposed battery connector wiring.

One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.   (a) Maintenance Shop: On or about April 15th, 2026, employees performed maintenance activities while utilizing a OTC 25 Ton Hydraulic Press ( Model 1833) without proper guarding.  (b)  High Speed Tray Sorter (HSTS) :  On or about April 16th, 2026, employees performed mail processing activities while utilizing several USPS Automatic Tray Unsleever (ATU) without proper guarding.

Dock Area (Mckinley Door 2) :  On or about April 15th 2026 , several JET PT-2742JA 5500 Lb 27" x 42" Pallet Truck (161008) that did not have the rated loads permanently marked on the equipment were in the pallet jack station near the truck dock area.
