29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provided for an alternative fall protection measure:  a) Job site, 330 John Street, Baldwin, PA: On or about April 21, 2026 during roofing activities, the employer did not ensure appropriate fall protection systems/devices were used where employees were exposed to fall hazards of up to approximately 20 feet.

29 CFR  1926.20(b)(2): A program was not provided for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers:    a) Job site, 330 John Street, Baldwin, PA: On or about April 21, 2026 during roofing activities, the employer did not ensure a system of hazard identification and correction was implemented at the site. Deficiencies noted during the inspection included, but were not limited to, not using fall protection, failing to use head protection at ground level, working near the electrical service drop with no insulating blanket on it and not having the ladder properly extended above the roofline.

29 CFR  1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails were not extended at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that would not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder:   a) Job site, 330 John Street, Baldwin, PA: On or about April 21, 2026 during roofing activities, the employer did not ensure the ladder used to gain access to and from the roof was properly extended above the landing area in that it was only about 30 inches above the roof line.
