29 CFR  1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  On or about April 17, 2026, the employer did not ensure the development, documentation, and utilization of energy control procedures to manage hazardous pneumatic energy during maintenance of air-actuated valves in the maintenance shop. Employees were required to apply compressed air to seat the valve stem; however, the employer did not establish or enforce procedures to control the application of pneumatic energy or ensure employees were clear of the point of operation during re-energization. Employees were permitted to connect compressed air directly to the valve without a required method of control or securing the valve, exposing them to caught-between hazards when the valve stem actuated, resulting in the amputation of an employee's right index fingertip.
