29 CFR  1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:   a) On or about April 22, 2026, at 638 Edgewood Street NE, Washington DC. Ground level southside/front; The employees were using extension cords that were visibly damaged to power various equipment, including saws, which placed them at significant risk of electrical shock. The damaged sections of these cords revealed exposed damaged internal wires, and internal twisting creating an unsafe working environment.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification. This documentation may include but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records

29 CFR  1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  a) On or about April 22, 2026, at 638 Edgewood Street NE, Washington DC. Ground level southside/front; The employees were using extension cords in which the strain relief was visibly damaged to power various equipment, including saws, which placed them at significant risk of electrical shock. The damaged sections of these cords revealed exposed damaged internal wires creating an unsafe working environment.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification. 29 CFR  1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:  a) On or about April 22, 2026, at 638 Edgewood Street NE, Washington DC, Employees were exposed to a fall hazard of approximately 30 feet to the ground below while performing siding installation work from an unguarded pump-jack system on a residential construction project. The employees were not properly secured/tied off, not using the "D" ring, exposing employees to the risk of injury and/or falling from elevated work surfaces during the performance of their assigned duties.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification. 29 CFR  1926.452(j)(2): Poles were not secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary.  a) On or about April 22, 2026, at 638 Edgewood Street NE, Washington DC; Employees while working on a pump-jack were exposed to an estimated 30-foot fall due to not installing the triangular bracing at the bottom of the poles to prevent from displacement while conducting siding work. 29 CFR  1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration:; Such activities were not performed only by experienced and trained employees selected for such work by the competent person.  a) On or about April 22, 2026, at 638 Edgewood Street NE, Washington DC; the pump-jack scaffold erected at the location was not erected by a competent person who understands the hazards associated with scaffolds and means to abate those hazards. The employees working on the scaffold were exposed to several hazards such as but not limited to fall hazards, and equipment displacement.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.
