29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about April 22, 2026, at the location of a worksite located at 1646 Wagner Ave., Lakewood, OH: The employer did not ensure that employees working at the roof porch level were protected from overhead hazards while roof work was being performed on the upper levels.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  On or about April 22, 2026, the employer did not ensure employees engaged in preparing a roof for shingle installation with a 10/12 pitch at a residential home located at 1646 Wagner Ave., Lakewood, OH 20-25 feet above the ground were protected by guardrail systems, safety net system, or personal fall arrest system.  Gilberto Roofing, LLC, was previously cited for a violation of this occupational safety and health standard, 1926.501(b)(13) , which was contained in OSHA inspection number 1866210, citation number2 , item number 2 and was affirmed as a final order on 2/17/2026, with respect to a workplace located at 3583 Warren Road, Lakewood, OH.  Gilberto Roofing, LLC, was previously cited for a violation of this occupational safety and health standard  1926.501(b)(13) , which was contained in OSHA inspection number 1830778, citation number1 , item number 2 and was affirmed as a final order on 7/17/2025, with respect to a workplace located at 1603 Clarence Ave., Lakewood, OH.

29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about April 22, 2026, at the location of a worksite located at 1646 Wagner Ave., Lakewood, OH: employees were exposed to a fall hazard of approximately 20 feet while performing roofing activities on a residential home . Employees were using an extension ladder to access the roof, and the employer did not ensure they extended at least 3 feet above the upper landing surface.  Gilberto Roofing, LLC, was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.1053(b)(1 , which was contained in OSHA inspection number 1830778, citation number1 , item number 3 and was affirmed as a final order on 7/17/2025, with respect to a workplace located at 1603 Clarence Ave., Lakewood, OH.
