29 CFR  1910.212(a)(1):  One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  Production workers using the motorized exit rollers in the Truss shop building located at 2302 E. Wilson, Ottawa, Kansas were exposed to struck-by hazards due to no machine guarding being provided for the north side rotating belts/pulleys.

29 CFR  1910.1200(e)(1): The employer did not develop, implement and maintain at the workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g) and (h) of this section for labels and other forms of warning, material safety data sheets, and employee information and training will be met:  At the workplace located at 2302 E. Wilson, Ottawa, Kansas, employees were exposed to health hazards due to no written hazard communication program developed nor implemented to address the hazardous chemicals used.  Chemicals used onsite included but not limited to propane, gasoline, Rea Jet ink, chain/cable fluids, among others.

29 CFR  1910.1200(h)(1): The employer did not provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new physical or health hazard the employees had not previously been trained about was introduced into their work area:  At the workplace located at 2302 E. Wilson, Ottawa, Kansas, employees were exposed to health hazards in that the employer did not train the employees on how to recognize, evaluate, mitigate, and control exposure to chemical hazards. Chemicals used onsite included but not limited to propane, gasoline, Rea Jet ink, chain/cable fluids, among others.

29 CFR  1910.178(q)(7):  Industrial trucks were not examined before being placed in service, and shall not be placed in service if the examination shows any condition adversely affecting the safety of the vehicle. Such examination shall be made at least daily:  Employees operating  powered industrial trucks at the facility located at 2302 E. Wilson, Ottawa, Kansas, were exposed to struck-by hazards in that pre-inspection examinations were not being performed.
