29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles.  (a) On or about April 28, 2026, at 1782-1788 Horseshoe Lane Slinger, WI 53086, the employer did not ensure employees conducting framing operations, using hand tools and nail guns were wearing eye protection. This exposed the employees to struck by hazards and eye injuries.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  (a) On or about April 28, 2026, at 1782-1788 Horseshoe Lane Slinger, WI 53086, employees installing sheathing were not using fall protection. This exposed employees to a fall hazard of approximately 21 feet 2 inches from the back of the home to the ground below.

(a) On or about April 28, 2026, at 1782-1788 Horseshoe Lane Slinger, WI 53086, the employer did not  document fall protection training with a written certification record.
