29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven (7) feet or less above floors or platforms were not enclosed: On or about April 3, 2026, the employer did not ensure that the chain and sprocket drive mechanism associated with the Castor Oil Line #2 conveyor system was enclosed to prevent employee contact. The chain and sprocket were located less than seven (7) feet above the working surface and were not enclosed. Employees working in proximity to the conveyor system were exposed to contact with the moving chain and sprocket during operation, and an employee contacted the mechanism and sustained amputation injuries to two fingers of the employee's hand.
