29 CFR  1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a. 183 Sheridan Ave HoHoKus , NJ - Roof  The employer did not ensure that employees were protected from fall hazards.  Employees engaged in residential roofing activities worked at or near the edge at heights greater than 20 feet and used Werner extension ladders for roof access where the ladder side rails did not extend at least 3-feet above the upper landing surface. Violation observed on or about 04/29/2026   Quetzaltenango Construction LLC was previously cited for serious violations of this Occupational Safety and Health standard or its equivalent standard, 29 CFR 1926.1053(B)(1): which was contained in OSHA Inspection Number 1742747, Citation # 1, Item # 2, issued on 07/08/2024 with respect to a workplace located at 2 Hillas Lane, Chester NJ 07930. This citation became a final order on 12/05/2025.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a) 183 Sheridan Ave, Ho Ho Kus NJ: Employees engaged in roofing activities on an 11 on 12 pitch were exposed to fall hazards of approximately 20 feet to the asphalt below as a means of fall protection was not being utilized. Condition observed on or about 04/29/2026.
