(a) On April 28, 2026, at or near 2716 Second Avenue West in Williston, North Dakota, employees were exposed to fall hazards of over 18 feet while installing roof sheathing material on a commercial roof without using a fall protective system.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) On April 28, 2026 at or near 2716 Second Avenue West in Williston, North Dakota, employees using an aluminum extension ladder to access and egress the roof deck were exposed to injuries from the displacement of an improperly installed ladder.
