29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: a) On or about 05/04/2026 in the Tipper/Tacker area; where the Tipper machine had a point of operation guard made of expanded metal, where the smallest opening in the guard was bigger than one-half inch and less than two inches from the point of operation (NOTE: according to Table O-10, guards having openings of this size would have to be at least 3-1/2 inches form the point of operation) ABATEMENT DOCUMENTATION REQUIRED
