29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:  (a) Commercial building, east upper and lower rooftops, various locations; On or about April 30, 2026, employees were not protected from falls while engaged in roofing replacement activities on 3 or 4 in 12 pitch roofs, at a height of approximately 12 feet (lower roof) to 20 feet (upper roof) above ground level.

29 CFR  1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  (a) Commercial building, east upper and lower rooftops; On or about April 30, 2026, the employer did not provide a written certification record of fall protection training for employees performing roofing replacement activities at a height of approximately 12 feet (lower roof) to 20 feet (upper roof) above ground level.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  (a) Commercial building, east side; On or about April 30, 2026, the side rails of a Werner extension ladder and a Gorilla multi-extension ladder, which were placed at ground level and leaned against the lower roof eave for employee access to the roof approximately 12 feet above ground level, did not extend at least 3 feet above the landing surface.
