Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: (a) Commercial building, east upper and lower rooftops, various locations; On or about April 30, 2026, employees were not protected from falls while engaged in roofing replacement activities on 3 or 4 in 12 pitch roofs, at a height of approximately 12 feet (lower roof) to 20 feet (upper roof) above ground level.
