29 CFR  1926.451(c)(3): Supported scaffold poles, legs, posts, frames, and uprights shall be plumb and braced to prevent swaying and displacement.   A) 84 Sprague Road, Scarsdale - NY: (Left Side of the House)  Employees were exposed to fall hazards of approximately 19 1/2 - feet while working on a pump jack scaffold that was not braced at the bottom. The employees had harnesses on but were tied of the the scaffold itself. The exposed employees were exposed to falls and/or scaffold collapse hazards, on or about, March 13th, 2026.  B) 84 Sprague Road, Scarsdale - NY: (Back of the house) An employee was exposed to fall hazards of approximately 13 1/2 - feet while working on a pump jack scaffold that was not braced at the bottom. The employee had a harness on but was not tied off to anything, nor was there an end-rail system or any other type of complete fall protection system on the scaffold. The exposed employee was exposed to falls and/or scaffold collapse hazards, on or about, March 13th, 2026.

29 CFR  1926.1053(b)(1):  Portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder is used to gain access:  A) 84 Sprague Road, Scarsdale - NY : An employee was exposed to fall hazards of approximately 13 1/2 - feet while using an extension ladder to access a pump jack scaffold. The extension ladder that was used, was only extended approximately 2-feet above the landing and not the recommended 3-feet as required by the manufacturers instructions, on or about,  March 13th, 2026.
