29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) On May 6, 2026, at the above addressed jobsite, employees engaged in cedar roof repair work were not protected from fall hazards while working at an elevation of approximately nine (9) feet six (6) inches above the ground.  No abatement certification or documentation is required for this item.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:   a) On May 6, 2026, at the above addressed jobsite, employees were exposed to fall hazards of approximately nine (9) feet six (6) inches to the ground where a portable extension ladder used by employees to access and egress the roof did not have its side rails extended at least 3 feet above the roof eave.   Cedar Preservation Specialists, Inc. dba Roof Max was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1780679, citation number 1, item number 2 and was affirmed as a final order on December 20, 2024, with respect to a workplace located at 730 Greenbay Road, Winnetka, Illinois 60093.  No abatement certification or documentation is required for this item.
