29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about May 12, 2026, employees were exposed to a fall hazard of over 11 feet while removing shingles from a residential home located at 5249 W 45th Street, PARMA, OH 44134. Employees were using an extension ladder to access the roof, and the employer did not ensure it extended at least 3 feet above the upper landing surface.
