29 CFR  1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following:  29 CFR 1910.147(c)(4)(ii)(B): Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy;  29 CFR 1910.147(c)(4)(ii)(C): Specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them; and  29 CFR 1910.147(c)(4)(ii)(D): Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures.  a. On or about May 11, 2026, employees were exposed to machine hazards associated with the Dal Prete Mini Compact Water Filtration System, located at the Shop Building of the worksite. The employer did not establish and implement specific procedural steps for shutting down, isolating and securing the water pumper to control the hazardous energy.  No specific procedures were established for the placement, removal and transfer of lockout/tagout devices, as well as requirements for testing the equipment to verify the effectiveness of the energy control measures.  As a result, an employee sustained injuries when their left index finger was struck and caught-in between the water pump valve opening and closing mechanism within the pump.

29 CFR  1910.147(c)(6)(i): The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed.  a. On or about May 14, 2026 times prior, the employer did not conduct periodic inspection of their  energy control procedure at least annually to ensure that the procedures and the requirements of the standard were adhered to.  The condition exposed authorized employees to machine hazards associated with the Dal Prete Mini Compact Water Filtration System, Model: Mini Compact M 2.1, S/N 137-22, as a result.

29 CFR  1910.147(d):  Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:  a. On or about May 11, 2026, employees were exposed to machine hazards associated with the water pump associated with the Dal Prete, Model: Mini Compact M 2.1, S/N 137-22, when they performed service and maintenance tasks for the water pump.  The employer failed to implement energy control application steps as LOTO device was not applied for the Dal Prete, Mini Compact M 2.1 [per 1910.147(d)(1) requirements.  As a result, the remaining applicable energy control elements, involving the shutdown of the machine or equipment [(d)(2)]; machine isolation [(d)(3)]; LOTO device application [(d)(4)]; dissipation of residual energy [(d)(5)(i)] and verification of isolation [(d)(6)], were not implemented to protect the employees from machine maintenance hazards.  An employee was injured when their left index finger was struck-by and caught-in between the valve mechanism within the water pump as a result.
