29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms: a) Job site, 5256 U.S. Route 30, Greensburg PA: On or about May 11, 2026 during masonry work, the employer did not ensure appropriate fall protection was in use along the ends of scaffolding outriggers, exposing employees to fall hazards of up to approximately 14 feet. b) Job site, 5256 U.S. Route 30, Greensburg PA: On or about May 11, 2026 during masonry work, the employer did not ensure appropriate fall protection was in use along the end of a section of scaffolding in that the bottom of a yellow end gate was at such a height that it could only serve as a top rail and no midrail was in use, exposing employees to a fall hazard of up to approximately 14 feet. c) Job site, 5256 U.S. Route 30, Greensburg PA: On or about May 11, 2026 during masonry work, the employer did not ensure appropriate fall protection was in use along the side of the scaffolding where the mud tub was located in that the existing cross bracing and the rail directly below it, was at such a height that it could only serve as a top rail and no midrail was in use, exposing employees to a fall hazard of up to approximately 14 feet.

29 CFR 1926.20(b)(2): A program was not provided for frequent and regular inspections of the job sites, materials, and equipment to be made by a competent person designated by the employer: a) Job site, 5256 U.S. Route 30, Greensburg PA: On or about May 11, 2026 during masonry work, the employer did not ensure a system of hazard identification and abatement was implemented at the job site. Deficiencies noted during the inspection included, but were not limited to, failing to use proper fall protection on scaffolding, having tripping hazards on the scaffold due to a board raising upwards and using a board(s) at ground level to cover a depression which bowed downwards due to lack of strength/rigidity and that curled upwards at the ends causing trip hazards.
