29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: Location 856 Washington St, Braintree, MA On May 19, 2026, workers were exposed to fall hazards of up to 40 feet to the ground below while conducting roofing activities on a 7/12 steep pitch roof where fall protection was not used
