29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: Location 856 Washington St, Braintree, MA On May 19, 2026, workers were exposed to fall hazards of up to 40 feet to the ground below while conducting roofing activities on a 7/12 steep pitch roof where fall protection was not used. Fall protection training was not provided to recognize and minimize fall hazards.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: Location 856 Washington St, Braintree, MA On May 19, 2026, workers were exposed to fall hazards of up to 40 feet to the ground below while conducting roofing activities on a 7/12 steep pitch roof where fall protection was not used F J General Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(1), which was contained in OSHA inspection number 1676488, citation number 1, item number 1 and was affirmed as a final order on December 12, 2023, with respect to a workplace located at 225 Turnpike St, Canton, MA.
