29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system:   810 Southampton Road, Westfield, MA: On or about May 26, 2026, four employees were observed working on a residential structure engaged in residential construction roofing activities with unprotected sides and edges greater than 6 feet above the ground below and were not protected by guardrails, safety net systems, or personal fall arrest systems.  Sandro's General Construction Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1759522, citation number 1, item number 2 and was affirmed as a final order on December 4, 2024, with respect to a workplace located at 21 Oak Street, Methuen, MA.

29 CFR  1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer:  810 Southampton Road, Westfield, MA: On or about May 26, 2026, the employer had not prepared a written certification record of fall protection training which included the name or other identity of the person trained,  the signature of the person trained, date(s) the training was completed and the signature of the person who conducted the training.
