29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a.  On or about May 27, 2026, at the above addressed jobsite, employees using pneumatic nail guns to install roof shingles were not utilizing eye protection, thereby exposing the employees to hazards associated with eye injuries.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):   a.  On or about May 27, 2026, at the above addressed jobsite, employees performing residential roofing operations, eighteen (18) feet eleven (11) inches above ground, were not protected from falls, thereby exposing the employees to fall hazards.  b.  On or about May 27, 2026, at the above addressed jobsite, an employee performing residential roofing operations, nine (9) feet seven (7) inches above ground, was not protected from falls, thereby exposing the employee to fall hazards.  c.  On or about May 27, 2026, at the above addressed jobsite, an employee performing residential roofing operations, eight (8) feet four (4) inches above ground, was not protected from falls, thereby exposing the employee to fall hazards.

29 CFR  1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  a.  On or about May 27, 2026, at the above addressed jobsite, employees performing framing carpentry operations were provided fall protection training; however, the employer failed to verify the training by preparing a written certification of training record.
