29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    (a) On or about May 27, 2026, employees conducting roofing activities on a residential structure to include tear-off  and shingle installation, were not protected from falling to the lower level by a personal fall arrest system, or other means of fall protection. This exposed employees to a fall hazard of approximately 22-feet at the eave and up to  approximately 28 feet at the gabled  ridge  onto grass, and concrete.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access.  (a) On or about May 27, 2026, the 2 portable extension ladder(s) being used by the employees to access the 2nd story roof did not extend at least three feet above the upper point of contact with the roof, nor were the ladders secured in place. This exposed employees to a fall hazard of 22-feet onto grass and concrete.  (b) On or about May 27, 2026, the portable articulating ladder being used by the employees to access the porch roof did not extend at least three feet above the upper point of contact with the roof, nor were the ladders secured in place. This exposed employees to a fall hazard of 13-feet onto grass and concrete.

29 CFR  1926.503(b)(1): The employer did not verify fall protection training by preparing a written certification record.  (a) On or about May 27, 2026, the employer did not provide written documentation of  fall protection training for exposed employees working from fall heights of between 13-feet and 28-feet on a steep pitched roof without being tied in to an adequate fall protection system.
