29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms:  (a) Single-family home exterior, east side; On or about May 27, 2026, an employee working from a pump jack scaffold system to install siding panels and perform other exterior construction activities was not protected from falling from heights up to approximately 19 feet 4 inches above ground-level as the scaffold system was not equipped with a mid-rail, screen or mesh along the length of the work platform, end stops were not in place, and alternative fall protection measures were not in use.

29 CFR 1926.452(j)(2): Poles were not secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary:  (a) Single-family home exterior, east side; On or about May 27, 2026, an employee working from a pump jack scaffold system to install siding panels and perform other exterior construction activities at heights up to approximately 19 feet 4 inches above ground-level was exposed to fall and structural instability hazards associated with an improperly secured work platform, in that the vertical poles supporting the system were not provided with the required bottom braces to prevent swaying or displacement, creating the potential for the scaffold to shift or collapse during use.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:  (a) Single-family home exterior, east side; On or about May 27, 2026, an employee working from a pump jack scaffold system to install siding panels and perform other exterior construction activities had not been trained by a qualified person to recognize the hazards associated with the scaffold, including but not limited to, fall hazards of up to approximately 19 feet 4 inches above ground-level due to the lack of a mid-rail and end stops, and scaffold instability due to the lack of bottom bracing.

29 CFR  1926.1053(b)(1):  Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) Single-family home exterior, east side; On or about May 27, 2026, a Mighty Multi brand model MMHL-22 multi-position aluminum ladder, configured as an extension ladder and used to access and egress a pump jack scaffold system, was not set-up with the side rails extending at least three feet above the upper landing surface and/or was not secured from displacement.
