On a roof, at a temporary jobsite located at 1073 Lido Road, in Jacksonville, Florida: On or about June 4, 2026, the employer exposed employees to eye injuries, in that eye protection was not utilized by employees while operating pneumatic nail guns.

On a roof, at a temporary jobsite located at 1073 Lido Road, in Jacksonville, Florida: On or about June 4, 2026, the employer exposed employees to fall hazards, in that fall protection, was not utilized by employees, while performing roofing work.
