29 CFR  1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.  a) On or about and at times prior to June 9, 2026, at the establishment located at 2905 North 15th Street, Sheboygan, Wisconsin; the employer did not ensure that the manual deli cheese slicer (Mfr: Hobart Corporation, Model: 2812, SN: 561-180-409) had a fence on the carriage tray to hold cheese being cut, requiring employees to place hands near the blade to hold cheese when slicing, instead of holding a carriage tray handle, exposing employees to amputation and laceration hazards. On May 28, 2026, while an employee was holding cheese on the manual deli cheese slicer (Mfr: Hobart Corporation, Model: 2812, SN: 561-180-409) when slicing, their finger came into contact with the blade, resulting in an avulsion.

29 CFR  1904.29(b)(1): What do I need to do to complete the OSHA 300 Log? You must enter information about your business at the top of the OSHA 300 Log, enter a one or two line description for each recordable injury or illness, and summarize this information on the OSHA 300-A at the end of the year.  ONEGUIDE, Inc. DBA Tietz's Piggly Wiggly - 2905 North 15th Street, Sheboygan, Wisconsin. On or about June 10, 2026, the employer did not provide the required injury and illness description on the OSHA Form 300 or equivalent for the calendar year 2026.  a) On May 28, 2026, while an employee was slicing cheese on the manual deli cheese slicer (Mfr: Hobart Corporation, Model: 2812, SN: 561-180-409), the employee sustained an avulsion injury to their right index finger (Case Number 1), which resulted in a job transfer or restriction case. The employer's OSHA 300 Form or equivalent for the calendar year 2026 did not contain a complete description of the object/substance that directly injured or made the person ill and an accurate description of the injury.
