On or about December 22, 2025, at the jobsite: employees were exposed to approximate 17-foot fall hazards while in the process of working on a 7:12 sloped roof, in that; fall protection was not used Southeast Hydrovac, LLC DBA Southeast Roofing and Construction, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1815587, citation number 1, item number 1 and was affirmed as a final order on July 02, 2025, with respect to a workplace located at 1000 Ocean Shore Blvd. Ormond Beach FL 32176.
