29 CFR 1910.178(l)(4)(ii)(B): Refresher training, including an evaluation of the effectiveness of that training, was not provided to the operator when the operator has been involved in an accident or near-miss incident: a) Worksite, 525 Neelytown Rd, Montgomery, NY 12549, Isle 33: On or about December 14, 2025 two employees were involved in an accident where a Crown Center-Control Rider Pallet Truck struck a pallet filled with cases of water causing multiple cases of water to fall, landing on the employee's left knee. Refresher training, including an evaluation of the effectiveness of that training was not provided to the employees involved in the accident.
