On or about June 16, 2026, employees were exposed to fall hazards of approximately 30 feet while performing residential roofing activities on the upper roof which had a steep pitch of 12:12.

On or about June 16, 2026, side rails of the portable extension ladder did not extend at least three feet above the roof's edge, exposing employees to fall hazards of approximately 30 feet to the ground below.
