29 CFR 1926.20(b)(2): A program was not provided for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employer: a) Job site, 3934 California Avenue, Pittsburgh, PA: On or about June 20, 2026 during roofing activities, the employer did not ensure a system of hazard identification and correction was in use at the site. Deficiencies noted during the inspection included, but were not limited to, failing to use eye/face protection while operating nail guns, working near an electrical service drop that was not blanketed, ladder did not have proper labeling and was set up on a tarp, debris not being cleaned up, head protection not being used by the person at ground level, failing to use fall protection and a ladder not being extended at least 3 feet above the landing.
