Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The

employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that:
a) Employees working in the Fabrication Shop of Lincoln Fabricating
company were not provided with a procedure by which equipment would
be reordered inoperative during maintenance or repair operation: to
prevent unintended or inadvertent startup or activation.
Among methods available to correct the condition, one feasible and
acceptable abatement method is the implementation of a locking and
lagging procedure which will render equipment being worked on to a
deenergized and zero mechanical state condition with no capability
stored energy or being subject to inadvertent or unintended start-up.
