Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to
hazard of crushing and lacerating injuries resulting from the
utilization of a 5/16" hoist wire rope on a 5-ton pendant controlled,
monorail hoist to lift and transfer loads which exceeded the breaking
strength of the wire rope on the west side of the cold end main line.
Among other methods one feasible and acceptable abatement method to
correct this hazard is to decrease the weight of the jumbo packs being
lifted by the 5/16" hoist wire rope.

or serious physical harm to employees in that employees were exposed to
hazards of fire and explosion while closing and adjusting deteriorated
and or unsuitable furnace port plug valves using hammers, wrenches and
or pipes, on or about 5/31/85.
Among other methods one feasible and acceptable abatement method to
correct this hazard is to adopt and adhere to a program of operational
safety checks and preventative maintenance which includes the
replacement of unsuitable and or deteriorated furnace port valves both
main and orifice types, and routine lubrication of the valves.

or serious physical harm to employees in that employees were exposed to
hazards of crushing and lacerating injuries resulting from the operation
of a vacuum lifter which was suspended from a pendant controlled bridge
crane and was being utilized to strip individual lites of jumbo glass
packs in the New Warehouse while the red "no go" light on the operator's
control panel was lighted. Further, vacuum pressure was still indicated
on five of the six vacuum cup gauges when the vacuum lifter was
deenergized
after making a lift in the New Warehouse, on or about 5/15/85.
Among other methods one feasible and acceptable abatement method to
correct this hazard is to operate the vacuum lifter following the
manufacturers operating instructions written on the equipment.
