Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to death and/or crushing injuries from falling loads as a screw pin shackle was used without a key to prevent pin back out at the connection between the pill and becket on the whip line: a) Anderson Box - Screw pin shackle between the pill and becket on the 35 ton Grove hydraulic truck crane, serial number 32784, was not protected against shackle pin back out, on or about July 3, 1986. Among other methods, one feasible and acceptable abatement method to correct this hazard is by using a shackle with a key thru the screw pin.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a 21 foot fall hazard while working from an unsecured and unguarded work platform being elevated by a forklift: a) Anderson Box - Employee working off an elevated platform that was not firmly secured to the forks of the powered industrial truck.
